I give Down Syndrome Network (DSNetwork) and all photographers representing Down Syndrome Network (DSNetwork) permission to use, re-use, publish and republish the photographs taken of  my family member(s) and/or of me. I understand and agree to the use of my name and child’s/family members’ names in connection with the DSNetwork’s publications, website, social media and/or marketing, videos and/or images.  I waive the right to inspect or approve the finished photograph, video, written copy, public relations copy of printed matter that may be used or created and appear in connection therewith.  I understand and acknowledge that there will be no payment for my participation (article, picture, videotaping, etc.). I declare that I am at least 18 years of age and the subject of this release or that I am the legal guardian.
