Thank you for registering to be a Buddy for Buddy Up Tennis. I understand that photos may be taken of participants and volunteers and used on DSNetwork social media and website and promotion purposes.

Upon completion of the registration form, I agree to complete and submit the Buddy Up for Life's Become a Buddy Application and Background Check.

(Note: Returning volunteers to the Buddy Up Tennis program, those under the age of 18 and those with a current Fingerprint Clearance Card do not have to complete a Background Check.)
