CANCELLATION POLICY
By checking the box below, I promise to do my best to attend this event. I understand that seats will be reserved for me and will notify DSNetwork with at least 72 hours notice if I need to cancel.
LIABILITY WAIVER
I understand that my participation in this event is voluntary. In consideration of me and/or my minor child being permitted to participate, I hereby-for-myself and personal representatives assume any and all risks which might be associated with the event. I further waive, release, discharge and covenant not to sue Down Syndrome Network, and any event partners, its officers, employees, organizers, volunteers, or other representatives, or their successors and assigns, for any and all injuries or damages of any kind whatsoever suffered by myself and/or my minor child as a result of taking part in the event and related activities.
PHOTO RELEASE
I also authorize the use by DSNetwork of any photo, film or video taken of me and/or my minor/adult child at the event for publicity and any purpose.
