In consideration of me and/or my minor/adult child being permitted to participate in Adaptive Zumba, I hereby-for-myself and personal representatives assume any and all risks which might be associated with the event. I further waive, release, discharge and covenant not to sue Down Syndrome Network and Susan Alonzo, its officers, employees, sponsors, organizers, volunteers or other representatives or their successors and assigns, for any and all injuries or damages of any kind whatsoever suffered by myself and/or my minor child/adult as a result of taking part in the event and related activities. I authorize the use by Down Syndrome Network of any photo, film or video taken of me and/or my minor/adult child at the event for publicity and any purpose.I also authorize to receive text messages from DSNetwork knowing I can text STOP to opt out at any time.
