COMMUNITY GARDEN WAIVER OF LIABILITY
RELEASE AND WAIVER:
I am aware of the risks and hazards that may arise through participation in the DSNetwork Community Gardening Program and assume any expenses and liabilities that I, my child or person under my guardianship incur in the event of an accident, illness, or other incapacity. Possible outdoor garden risks include but are not limited to: insects, animals, tool use, weather exposure, lifting, bending, walking on uneven terrain, etc. I hereby waive and release any and all rights and claims for damages against DSNetwork, its employees, representatives and any individual associated with DSNetwork, and will hold them harmless for any and all injuries suffered in connection with activities at the Community Garden.
PHOTOGRAPHIC RELEASE:
Further, I hereby grant full permission to any and all of the foregoing to use my likeness in all media including photographs, recordings, or any other record made in connection with my activities at the Community Garden.
PLEASE NOTE: If a garden participant is under 18 years of age or under legal guardiaship, then a parent or guardian must sign this waiver on their behalf.
Under no circumstances will any participant be allowed in the community garden without first signing this Waiver of Liability Form.
